If you make a $75 purchase through my Beautycounter link/website from December 7th – 11th, 2016, I will personally send you a necklace or earrings of your choice! There are 6 different options*:

STEP 2: Complete this form with your order #, mailing address, and jewelry selection no later than 11:59pm PT on December 12, 2016.
